Health promotion and disease prevention; diagnosing and managing chronic psychosocial and physiological alterations in adult and geriatric patients.

Uses Health Assessment and Diagnostic Testing

Assesses chronic/geriatric health problems commonly seen in adults in primary care settings using appropriate health assessment skills and utilizing applicable diagnostic testing.

Evaluates Evidence for Screening, Differential Diagnosis, and Management

Evaluates the latest/current evidence base for screening, differential  diagnosis, and management of chronic/geriatric health problems. 

Develops Therapeutic Health Plans

Develops therapeutic health plans for chronic/geriatric conditions using pharmacologic and non-pharmacologic therapeutic modalities. 

Provides Health Promotion and Disease Prevention Education

Provides appropriate health promotion and maintenance and disease prevention patient education that is culturally appropriate and considers the patient's social, racial, educational, financial, and family needs.
